Inter-Organizational Collaboration



Our community-focused implementation process strengthens the link between health, social service, and education professionals. We accomplish this by working at the neighborhood level in close collaboration with pre-existing community organizations to directly intervene against the cause of health disparities; low neighborhood wealth, inadequate social support, lack of public infrastructure, neighborhood safety concerns, and lack of access to primary care.

This process includes steps designed to establish/improve pre-existing inter-organizational partnerships, increase/refine the communities understanding of the social determinants of health at the neighborhood level, cooperate/engage in the development a coherent community focused strategy, measure/monitor impact of the initiative over time, and disseminate lessons learned to throughout the community.
