About Heudia

Closing the Gaps
in Human-Centered Care

We believe every person deserves access to the right care, at the right time, regardless of where they live, what they earn, or what challenges they face. AccessMeCare™ is how we make that real.

Origin Story

It started with a child's health.
It grew into a movement.

Heudia was born from a deceptively simple question: why are the people who need care the most, the hardest to reach? Our founder, Ed Connors — a public health innovator nominated for the National Medal of Technology and Innovation — set out to answer it through a federal research contract focused on childhood obesity. What he discovered was far bigger than nutrition. It was a broken system of invisible walls.

Culture, distrust, fear, language, transportation, income — the same handful of barriers, again and again, standing between vulnerable people and the care that could change their lives. No single provider, clinic, or agency could see the full picture. And no technology existed to connect them all.

"AccessMeCare does for millions of people what we would all do for a friend or family member in a health care crisis — assess their needs, find the right help, and empower them to take action."

What We Believe

Identifying a need is not enough. The system must help them get there.

Human-centered care is adversely impacted by poor care transitions because of community-level factors, home-based issues, and individual behaviors. Our approach begins with a foundational conviction: Heudia's platform goes further than any alert or screening tool — it closes the loop, navigating individuals through the barriers that have always been there, just never addressed together.

So we built it ourselves. AccessMeCare™ was designed not as a referral tool, but as a living network of care — connecting medical providers, community-based organizations, behavioral health services, and the individuals themselves through a single, shared workflow. Not a handoff. Partnerships.

But the platform alone was never the answer. From the beginning, we understood that lasting change in a community happens when the people closest to the problem — neighbors, faith leaders, health workers, and local organizations — are treated as partners in building the solution, not simply recipients of it. We listened before we built. We actively convene the voices that rarely sit at the same table: the clinic, the food pantry, the housing navigator, the school nurse. And we design our technology around the relationships they already have, strengthening the lines of communication and mutual trust that hold a community's health together.

A Moment That Changed Everything

One grandmother. One navigator. One company.

At a Head Start wellness fair in Northeast Pennsylvania, an AccessMeCare navigator was helping Joan find eyeglasses for her five-year-old granddaughter, Victoria. Mid-conversation, Joan paused. She stated she had a cancerous lesion on her nose she'd been living with for over a year — spreading, untreated, and frightening. She didn't know where to go. She had no insurance, no money, and was afraid to ask for help.

Within minutes, the navigator identified four barriers to care and connected Joan to resources she didn't know existed. Moments like this is why Heudia exists.

We envision a healthcare ecosystem where every individual — regardless of income, zip code, or circumstance — can access the services they need when they need them most. AccessMeCare™ is our vehicle for closing the gaps that fragment care and leave vulnerable people behind.

01

Equity as the Starting Point

We design every feature for the hardest cases first — justice-involved individuals, those experiencing homelessness, and communities with the fewest resources.

02

Whole-Person, Whole-Community Care

True health encompasses housing, food security, mental wellness, and social connection — not just clinical diagnoses. Our platform coordinates across all of it.

03

Stronger Systems, Healthier Communities

By connecting providers, agencies, and individuals in a unified coordination layer, we strengthen the entire ecosystem — reducing duplication, waste, and avoidable harm.

04

Impact You Can Measure

Every person helped, every referral completed, every transition managed — tracked in real time so partners and communities can see exactly what's working.

Meet the Team

The people behind
AccessMeCare™

Heudia is a diverse team of public health strategists, technologists, data scientists, behavioral experts, and community advocates united by a common cause. We are led by people who have spent careers working inside the gaps — and who decided to build something that could finally close them.

We are proud to have earned the trust of federal agencies, community health centers, managed care organizations, rural health systems, and the families they serve. We operate with purpose, pride, and persistence — because the people depending on us deserve nothing less.

From a grandmother at a wellness fair in South Carolina to a pregnant mother in a rural county with no prenatal resources nearby, AccessMeCare™ has changed what it means to be seen by a system that once overlooked you.
