It started with a child's health.

Heudia was born from a deceptively simple question: why are the people who need care the most, the hardest to reach? Our founder, Ed Connors — a public health innovator nominated for the National Medal of Technology and Innovation — set out to answer it through a federal research contract focused on childhood obesity. What he discovered was far bigger than nutrition. It was a broken system of invisible walls.
Culture, distrust, fear, language, transportation, income — the same handful of barriers, again and again, standing between vulnerable people and the care that could change their lives. No single provider, clinic, or agency could see the full picture. And no technology existed to connect them all.

Human-centered care is adversely impacted by poor care transitions because of community-level factors, home-based issues, and individual behaviors. Our approach begins with a foundational conviction: Heudia's platform goes further than any alert or screening tool — it closes the loop, navigating individuals through the barriers that have always been there, just never addressed together.
So we built it ourselves. AccessMeCare™ was designed not as a referral tool, but as a living network of care — connecting medical providers, community-based organizations, behavioral health services, and the individuals themselves through a single, shared workflow. Not a handoff. Partnerships.
But the platform alone was never the answer. From the beginning, we understood that lasting change in a community happens when the people closest to the problem — neighbors, faith leaders, health workers, and local organizations — are treated as partners in building the solution, not simply recipients of it. We listened before we built. We actively convene the voices that rarely sit at the same table: the clinic, the food pantry, the housing navigator, the school nurse. And we design our technology around the relationships they already have, strengthening the lines of communication and mutual trust that hold a community's health together.

At a Head Start wellness fair in Northeast Pennsylvania, an AccessMeCare navigator was helping Joan find eyeglasses for her five-year-old granddaughter, Victoria. Mid-conversation, Joan paused. She stated she had a cancerous lesion on her nose she'd been living with for over a year — spreading, untreated, and frightening. She didn't know where to go. She had no insurance, no money, and was afraid to ask for help.
Within minutes, the navigator identified four barriers to care and connected Joan to resources she didn't know existed. Moments like this is why Heudia exists.
